Terms & Agreement

Agreement to Conditions:

Please acknowledge that you accept the following conditions:
- Requests must be submitted at least three (3) weeks prior to the date of the event.
- Submitting a request does NOT guarantee Hendrix will make an appearance; you will be contacted via email/phone with either a confirmation or a "not available" notice.
- Hendrix the Husky & the Center for Student Involvement have the right to refuse, deny, or cancel a scheduled appointment due to a scheduling conflict, illness, safety concerns, or other reasons deemed acceptable by administration.
- Further event details may be required after submitting this form, especially for off-campus events, in which case we will contact you or the day-of contact. Details such as dressing room availability, security, media presence, and billing address may be requested.
- Please remember that Hendrix is a student, and as such may not be available for your requested event. Hendrix's schedule fills up quickly so their appearances may be limited.
- While Hendrix may be able to attend events within the greater Tacoma community, priority will be granted to student and other UW Tacoma sponsored events first.
